St.
Barnabas Nursing Home offers specialized rehabilitation
services to individuals in order to improve and maintain
physical function, independence, and overall quality of life.
Licensed physical and occupational therapists provide
short-term and long-term rehabilitation services to residents,
including those who are geriatric, ventilator dependent, have
HIV/AIDS, and/or registrants who participate in the Adult Day
Care Program.
Goals
of the Rehabilitation Program:

Restoration of function

Teaching of compensatory strategies for the management of
disability

Prevention of further limitations from the secondary effects
of a medical condition

Maximization of functional abilities to improve overall
quality of life.
Physical Therapy Services:
-
Evaluation of needs initiated within 24 hours of admission
to St. Barnabas Nursing Home

Key
member of the interdisciplinary team and plan of care

Ambulation and gait training to improve walking and mobility
-
Transfer training to improve movement from one surface to
another
-
Strengthening, endurance training, and general conditioning
-
Pain
management
-
Wound
care and management
-
Community re-entry skills training
-
Resident and family/caregiver education
Occupational Therapy Services:
-
Evaluation of needs initiated within 24 hours of admission
to St. Barnabas Nursing Home

Key
member of the interdisciplinary team and plan of care

Self-care skills training
-
Wheelchair mobility training
-
Cognitive/perceptual training to improve orientation,
memory, and problem solving
-
Transfer training for tub/shower and toilet
-
Upper
extremity strengthening and conditioning

Pain
management

Community re-entry skills training

Resident and family/caregiver education
Short-Term Rehabilitation Program:
-
Targets individuals who have been discharged from the acute
hospital setting and continue to require intensive
rehabilitation services for a limited period of time in
order to return to the community environment.

One-to-one treatment provided by physical and occupational
therapy 5 – 6 days per week, 30 – 60 minutes per session.
-
Length
of stay may be from 2 weeks to 3 months.

The
overall goal of this program is to maximize the functional
abilities of the individuals admitted so that they may
return to and resume their lives in the community.
Long
-Term Rehabilitation Program:
-
Targets residents who require less intensive rehabilitation
in order to maximize their functional ability in the nursing
home setting.

Both
restorative and maintenance programs are available.
-
Restorative, one-to-one treatment provided by physical
and/or occupational therapy 5 days per week, 30 minutes per
session.
Adult
Day Care Rehabilitation Program:
-
Targets registrants who require rehabilitation services to
improve their physical function in order to remain in the
community.

Restorative, one-to-one treatment provided by physical
and/or occupational therapy 2 - 3 days per week, 30 minutes
per session.

Treatment provided on-site.
